About the Role

This is an exciting opportunity to work in a dynamic and innovative environment, where you will have the chance to make a real difference in the lives of our service users. As a Primary Care Mental Health Practitioner, you will be responsible for assessing and managing the mental health needs of individuals in primary care settings, working closely with GPs and other healthcare professionals to develop and implement effective care plans.

Key Responsibilities
  • Conduct comprehensive mental health assessments and develop individualized care plans in collaboration with GPs and other healthcare professionals.
  • Provide evidence-based interventions and treatments to individuals with mental health needs, including those with complex and severe mental health conditions.
  • Work closely with GPs and other healthcare professionals to ensure seamless transitions of care and effective communication.
  • Develop and maintain strong relationships with primary care teams, mental health professionals, and other stakeholders to ensure effective collaboration and coordination of care.
  • Participate in multidisciplinary team meetings and contribute to the development of mental health services and policies.
Requirements

To be successful in this role, you will need to have:

  • Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC).
  • Significant post-qualifying experience in mental health, preferably in a primary care setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals from diverse backgrounds and with complex mental health needs.
  • Strong assessment and intervention skills, with the ability to work independently and as part of a team.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
