About the Role

We are seeking a skilled Primary Care Mental Health Practitioner to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a key member of our primary care team, you will work closely with GPs and other healthcare professionals to provide mental health assessments and support to individuals in need.

Key Responsibilities

  • Conduct comprehensive mental health assessments and provide evidence-based interventions to individuals with mild to moderate mental health difficulties.
  • Work collaboratively with GPs and other healthcare professionals to develop and implement effective care plans.
  • Provide education and support to GPs and other healthcare professionals on mental health issues and best practices.
  • Develop and maintain strong relationships with primary care teams and other stakeholders to ensure seamless care coordination.
  • Contribute to the development and implementation of new services and initiatives to improve mental health outcomes in primary care.

Requirements

  • Registered mental health professional with significant post-qualifying experience in the relevant care group.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and teams.
  • Ability to work autonomously and as part of a team, with a strong focus on collaboration and partnership working.
  • Knowledge of mental health issues, policies, and procedures, with a commitment to evidence-based practice.
  • Ability to travel across sites and work in a fast-paced environment.
