Primary Care Mental Health Specialist

We are seeking a highly skilled and experienced Band 6 Primary Care Mental Health Practitioner to join our team at Cambridgeshire & Peterborough NHS Foundation Trust. As a key member of our Primary Care Mental Health Service, you will play a vital role in providing high-quality mental health care to individuals in primary care settings.
Key Responsibilities- Work closely with GPs and other primary care professionals to assess and manage mental health needs of patients.
- Provide specialist mental health advice and support to primary care teams.
- Develop and implement care plans to promote recovery and well-being of patients.
- Collaborate with secondary care services to ensure seamless transitions and effective care coordination.
- Contribute to the development and implementation of mental health services and policies in primary care.
- Registered mental health professional with relevant experience and qualifications.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Strong assessment and intervention skills.
- Knowledge of mental health legislation and policies.
