Primary Care Mental Health Specialist

We are seeking a highly skilled and experienced Band 6 Primary Care Mental Health Practitioner to join our team at Cambridgeshire & Peterborough NHS Foundation Trust. As a key member of our Primary Care Mental Health Service, you will work closely with GPs and other healthcare professionals to provide high-quality mental health care to individuals in primary care settings.
Your expertise will be invaluable in facilitating mental health assessments, supporting GPs with joint decision-making, and signposting service users to appropriate services. You will also play a crucial role in developing links and fostering a culture of partnership between Primary Care Teams and our Adult Mental Health Services.
We are looking for someone who is passionate about making a difference in the lives of our service users and is committed to delivering excellent care. If you have a strong background in mental health, excellent communication skills, and a willingness to learn and develop, we would love to hear from you.
Key Responsibilities- Work closely with GPs and other healthcare professionals to provide mental health care to individuals in primary care settings
- Facilitate mental health assessments and support GPs with joint decision-making
- Signpost service users to appropriate services and support their transition from secondary care to primary care
- Develop links and foster a culture of partnership between Primary Care Teams and our Adult Mental Health Services
- Provide specialist advice and guidance to GPs and other healthcare professionals on mental health matters
- Registered mental health professional with significant post-qualifying experience in the relevant care group
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Strong background in mental health and experience of working with individuals with complex health problems
- Ability to travel across sites and work in a fast-paced environment
