Primary Care Mental Health Specialist

We are seeking a highly skilled and experienced Primary Care Mental Health Practitioner to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a key member of our primary care team, you will work closely with GPs and other healthcare professionals to provide high-quality mental health services to our patients.
About the RoleThis is an exciting opportunity to work in a dynamic and innovative service that is making a real difference in the lives of our patients. As a Primary Care Mental Health Practitioner, you will be responsible for providing mental health assessments, interventions, and support to patients in primary care settings. You will work closely with GPs, practice nurses, and other healthcare professionals to develop and implement care plans that meet the individual needs of our patients.
Key Responsibilities- Provide mental health assessments and interventions to patients in primary care settings
- Work closely with GPs and other healthcare professionals to develop and implement care plans
- Provide education and support to patients, families, and carers on mental health issues
- Collaborate with other healthcare professionals to ensure seamless care transitions
- Contribute to the development and implementation of mental health services in primary care
