About the Role

This is an exciting opportunity to work in a dynamic and innovative environment, where you will have the chance to make a real difference in the lives of our service users. As a Primary Care Mental Health Practitioner, you will be responsible for providing mental health assessments, interventions, and support to individuals with mild to moderate mental health difficulties. You will work closely with GPs to develop management plans and provide education and training to primary care staff.

Key Responsibilities
  • Provide mental health assessments and interventions to individuals with mild to moderate mental health difficulties
  • Work closely with GPs to develop management plans and provide education and training to primary care staff
  • Develop and maintain relationships with primary care teams, mental health professionals, and other stakeholders
  • Contribute to the development and implementation of mental health services in primary care
  • Participate in multidisciplinary team meetings and contribute to the development of care plans
Requirements

To be successful in this role, you will need to have:

  • Registration with the NMC or HCPC
  • Experience of working in primary care or a similar setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of mental health legislation and policies
