Job Summary

We are seeking a highly skilled and motivated Primary Care Mental Health Practitioner to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a key member of our primary care team, you will work closely with GPs and other healthcare professionals to provide high-quality mental health services to our patients.

About the Role

As a Primary Care Mental Health Practitioner, you will be responsible for providing mental health assessments, interventions, and support to patients in primary care settings. You will work closely with GPs, other healthcare professionals, and patients to develop and implement effective care plans that meet the individual needs of each patient.

Key Responsibilities
  • Provide mental health assessments and interventions to patients in primary care settings
  • Work closely with GPs and other healthcare professionals to develop and implement effective care plans
  • Provide support and guidance to patients and their families to help them manage their mental health
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving services
