About the Role

We are seeking a highly skilled and compassionate Tenancy Support Coordinator to join our team at The Forward Trust. As a key member of our Housing Support Team, you will play a vital role in empowering individuals with substance misuse issues to achieve stable and sustainable tenancies.

Key Responsibilities
  • Provide personalized, trauma-informed support to individuals experiencing tenancy-related issues linked to substance misuse.
  • Coordinate and deliver intensive support to create long-term sustainable tenancies, promoting independence and community integration.
  • Work closely with healthcare professionals to monitor and manage client health, minimizing risks and promoting holistic support.
  • Contribute to the achievement of targets set by Forward and local commissioners, including measuring outcomes and co-operative working relationships with partner agencies.
  • Flexibility is required, including some evening/weekend shifts and travel to other projects in your cluster to deliver services.
Requirements
  • Take an active role in implementing and achieving project objectives, building strong relationships with line management and head office.
  • Prioritize caseloads depending on crisis intervention and severity of need, escalating to the Team Leader as appropriate.
  • Seek opportunities for co-location with District Housing Teams and Housing Providers to benefit from specialist housing expertise.
  • Provide preventative support for individuals in treatment, entering or on a path to enter a housing crisis.
  • Coordinate and work alongside healthcare professionals to monitor and manage client health, minimizing risks.
