About the Role

We are seeking a highly skilled and dedicated Tenancy Support Coordinator to join our team at The Forward Trust. As a key member of our Housing Support Team, you will play a vital role in providing personalized, trauma-informed support to individuals experiencing issues with their tenancy linked to substance misuse.

Key Responsibilities
  • Coordinate and provide intensive support to create long-term sustainable tenancies
  • Promote independence and support individuals in maximizing connectivity with the local community and partner agencies
  • Provide holistic support, addressing issues such as basic living skills, social integration, education, and employment
  • Work closely with healthcare professionals to monitor and manage client health and minimize risks of harm
  • Contribute to the achievement of project objectives and targets set by Forward and local commissioners
Requirements
  • Strong relationships with line management and head office
  • Prioritize caseload depending on crisis intervention and severity of need
  • Build strong relationships with District Housing Teams and Housing Providers for co-location and specialist housing expertise
  • Provide preventative support for those in treatment entering or on a path to enter a housing crisis
  • Meet required KPIs to corresponding deadlines, as set out by the Service
