Job Title: Tenancy Support Coordinator

We are seeking a highly skilled and compassionate Tenancy Support Coordinator to join our team at The Forward Trust. As a key member of our Housing Support Team, you will play a vital role in providing personalized, trauma-informed support to individuals experiencing issues within their tenancy linked to substance misuse.

Key Responsibilities:
  • Coordinating and providing intensive support to create long-term sustainable tenancies
  • Promoting independence and social integration while supporting individuals to maximize connectivity with the local community and partner agencies
  • Providing holistic support to address any issues, either directly or by signposting and referring to other services
  • Working closely with healthcare professionals to monitor and manage client health and minimize risks
  • Contributing to the achievement of project objectives and targets set by Forward and local commissioners
Requirements:
  • Active role in implementing and achieving project objectives
  • Strong relationships with line management and head office
  • Prioritizing caseload depending on crisis intervention and severity of need
  • Flexibility to work evening/weekend shifts and travel to other projects in the cluster
