Nurse Clinician for Diabetes Management

Are you a motivated and experienced Diabetes Specialist Nurse looking for a new challenge? Connaught Resourcing is currently recruiting for a permanent position with a leading National Service provider, offering an exciting opportunity to join a nationwide Diabetes team.
This role involves working in primary care settings, including GP practices and community clinics, where you will be responsible for developing and implementing individualized care plans for patients with diabetes. You will also conduct regular health checks and provide education to patients on managing their condition.
We offer a competitive salary of £55,000-£65,000 per annum, dependent on experience, plus a range of benefits, including pension scheme, life assurance, and access to professional development opportunities.
Key responsibilities include:
- Assessing patient needs and developing personalized care plans
- Conducting regular health checks and monitoring patient progress
- Providing education and support to patients on diabetes management
To succeed in this role, you will need:
- Registered nurse qualification and current registration with the NMC
- Experience in diabetes management and patient education
