**Job Description:** Nurse Clinician - Outreach

We are seeking an experienced and skilled nurse clinician to join our outreach team. As a nurse clinician, you will play a key role in delivering high-quality patient care in our community settings.

The successful candidate will have relevant clinical experience, including that as a student or unqualified nurse. They will also possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities:

  • Delivering an outreach service to support the inpatient service
  • Working collaboratively with multidisciplinary teams to provide high-quality patient care
  • Developing and implementing effective care plans to meet the needs of patients

Person Specification:

  • RN with NMC registration
  • Teaching and assessing qualification
  • Speciality specific qualifications
