Specialist Dementia and Frailty Team Leader

About the Role
We are seeking an experienced Specialist Dementia and Frailty Team Leader to join our team in West Essex. As a key member of our Specialist Dementia and Frailty Service, you will be responsible for leading the service in Uttlesford in delivering high-quality care to patients with dementia and complex mental health needs.
Main Responsibilities
- Provide leadership to the Multi-Disciplinary Team to deliver excellent patient care.
- Develop and implement evidence-based interventions to meet the holistic needs of service users.
- Build robust relationships with key stakeholders to support integrated care pathways.
- Manage the team to ensure safe, effective, and therapeutic clinical services.
- Oversee general service management, including supervisions and workforce procedures.
- Support others in their learning and development related to dementia and complex mental health needs.
Requirements
- Ability to conduct complex assessments and knowledge of dementia and complex mental health needs.
- Experience working with external agencies to support service users.
- Good oral communication skills and clinical expertise.
