About the Role

We are seeking a highly skilled Registered Dementia Practitioner to join our community-based assessment team. As an experienced professional, you will be responsible for managing a caseload autonomously, with support and supervision.

Key Responsibilities:

  1. Comprehensive Assessment: Conduct thorough assessments of new referrals to inform care plans and interventions.
  2. Mental Health Evaluation: Deliver high-quality mental health assessments, focusing on dementia-related conditions.
  3. Care Planning: Participate in multi-disciplinary team meetings to ensure holistic approaches to client care.
  4. Dementia Diagnosis and Treatment: Assess and diagnose dementia-related conditions and other mental health concerns in older adults.

Requirements:

  1. Experience: Recent experience (within the past 2 years) in a dementia assessment team or memory clinic setting is essential.
  2. Qualifications: A qualified mental health practitioner (e.g., RMN, Social Worker, OT) with a professional qualification is required.
