Job Title: Senior Dementia Specialist Team Lead
\Description:
\- We are seeking a highly motivated and experienced professional to lead our Specialist Dementia and Frailty Team in West Essex.
- The successful candidate will be responsible for delivering high-quality community mental health services, ensuring the effective management of patients with dementia and complex mental health needs alongside age-related frailty.
- This is an exciting opportunity to join a dedicated team and contribute to the development of integrated care pathways and locality working.
Main Duties:
\- Provide leadership and strategic direction to the Multi-Disciplinary Team (MDT) in delivering the service.
- Develop and implement evidence-based interventions to address the holistic needs of service users, review care plans, and ensure timely discharge to primary care or other providers.
- Foster robust inter-agency relationships with key stakeholders to support the development of integrated care pathways and locality working.
- Coordinate and manage the team to provide a safe, effective, and therapeutic clinical service, ensuring high-quality community mental health services.
- Responsible for general service management, including supervisions, sickness absence, disciplinary procedures, and workforce policies.
- Support staff development and learning in relation to patient needs, particularly those with dementia and complex mental health needs alongside age-related frailty.
