**Location**:We are looking for Dementia Advisers across Mid and South Essex to work on exciting projects.

**Hours**:Full and Part time hours available.

There is no direct care element to the Dementia Adviser role, however we work passionately to provide advice, support and guidance to people affected by dementia. These roles will support people and help provide the tools and knowledge to make informed decisions about their future. These particular roles require people who are passionate about getting involved in communities and relish working in challenging environments.

These roles will be looking at providing targeted dementia support to those experiencing health inequalities, particularly those with low incomes, at risk or currently experiencing homelessness and those with multiple health conditions. Other projects that are being worked on currently are providing support to people with a diagnosis of Dementia with a successful discharge from hospital to ensure that people have the right support to return to their homes safely and with confidence.
- Offering a vital and compassionate advice service to support and guide those affected by dementia.
- Provide community based, face to face services for an increased level of support for more complex cases.
- Managing referrals, assisting clients in assessing their information and support needs in a person-centred manner.
- Providing knowledgeable and trusted information regarding services and benefits available and, where appropriate, assist clients in accessing them.
- Signposting clients to a choice of suitable other sources of help, where appropriate.
- Developing a proactive approach in reaching people with dementia and carers who may not otherwise access our services.

We are hoping to welcome an approachable and knowledgeable team of Dementia Advisors to work in the Essex area. We provide information and support in people’s homes and at locations across Essex.

We are looking for:

- Working experience of assessing people face to face with an ability to assess their needs.
- Deliver advice and guidance with a non-judgmental approach and outstanding communication skills.
- Strong organisational and time management skills to manage your caseload of clients effectively.
- Experience of working in a hospital or community environment
- Good IT skills and be able to travel independently across Essex.
- An understanding of dementia and the needs of those living with dementia and their carers would be an advantage but if you have had experience supporting with other challenges this would be taken into consideration.
