Advanced Home-Based Practitioner

We have an exciting opportunity for an Extended Scope Clinical Practitioner (ECP) to join our team at the Mid Hampshire Urgent Community Response and Frailty Hospital at Home Virtual Ward.
The successful candidate will work with a multidisciplinary team to deliver high-quality assessments, diagnosis, and therapies to patients in their own homes. Our focus is on providing hospital-level interventions and diagnostics as an alternative to hospital admission for patients with acute needs.

We aim to provide seamless care across the region by integrating services from Southern Health, Solent NHS Trust, Isle of Wight NHS Trust's community and mental health teams, and Hampshire CAMHS. This integration enables us to offer the right support in the right place at the right time.
Key Responsibilities- Work collaboratively with a consultant practitioner-led team to assess, diagnose, and treat patients in their homes.
- Deliver high-quality care that meets the needs of patients with complex conditions.
- Appropriate professional qualification allowing registration with the NMC or HCPC.
- Professional UK registration (e.g., NMC/HCPC).
- Degree-level award.
- Independent non-medical prescribing or pharmacology level 7 profession-relevant qualification.
