Occupational Therapy Specialist

We are seeking an experienced Senior Occupational Therapist to join our Urgent Community Response and Frailty Hospital at Home Virtual Ward.
Main Responsibilities- To assess and treat patients with decompensating physical health conditions, managing their function at home and preventing hospital admission.
- To work collaboratively with multidisciplinary teams, including Consultant Practitioners, Advanced Clinical Practitioners, and Allied Health Professionals.
- To develop and implement care plans, ensuring timely delivery of treatment and services.
- To lead and facilitate patient-focused case management, coordinating services across primary and secondary care.

Requirements and Qualifications- Qualified Occupational Therapist with HCPC registration.
- Experience working with people living with frailty, good knowledge of health and safety, and effective communication skills.
- Relevant clinical setting experience as a postgraduate.
This role offers a competitive salary range of £37,338 to £44,962 per annum, based on full-time hours. We also offer opportunities for career development, training, and collaborative working, as well as flexible working options.
