Job Description

We are seeking a Reablement Practitioner to join our Reablement Team in Hampshire. As a Reablement Practitioner, you will support adults over 18 who may require additional care and support, working towards enabling them to achieve independence and remain living at home.

The successful candidate will work independently, driving to service users' homes to deliver reablement interventions. You will also work towards enabling service users to achieve independence in daily living tasks and personal care, delivering hands-on care and writing up medication as needed.

Key responsibilities include:

">
  • Driving to service users' homes and working independently to deliver reablement interventions.
  • Enabling service users to achieve independence in daily living tasks and personal care.
  • Delivering hands-on care and writing up medication as needed.
  • Undertaking risk assessments and completing service users' goals.
  • Completing supervisions with Community Reablement Assistants Level 1s and supporting their development.
  • Using various IT systems such as MS Teams and email.

This is an exciting opportunity to work as part of a team providing support and guidance to individuals with short or long-term conditions and frailties. You will be required to work evenings and/or public holidays and/or weekends as part of your normal working hours, with enhancements paid in line with the HCC salary policy.

Salary Information

">

The estimated salary for this role is £24,009 - £30,615 per annum, depending on experience.
