Job Description: Advanced Healthcare Practitioner

Hampshire and Isle of Wight Healthcare NHS Foundation Trust seeks a skilled Advanced Healthcare Practitioner to join our Urgent Community Response and Frailty Hospital at Home teams. The post holder will work under supervision to develop expertise in assessment, diagnostics, and treatment of acutely unwell patients. They will also contribute to service development, applying quality improvement methodologies to promote high-quality practice.

About the Role:
This exciting opportunity requires a highly motivated individual with a strong background in clinical care. The post holder will work collaboratively with individuals, families, and carers to provide innovative solutions to enhance patient experience and improve outcomes. They will receive supervision and development from qualified advanced clinical practitioners and consultant practitioners. Estimated salary: £55,000 per annum. Flexible working options available.

Key Responsibilities:
- Assessment, diagnostics, and treatment of acutely unwell patients.- System working with individuals, families, carers, communities, and others for population health and reducing inequalities.- Development of specialized clinical skills across frailty, urgent response, and hospital at home care.- Contribution to service development using quality improvement methodology.- Collaborative working with multidisciplinary teams.- Emphasis on patient-centered care and empowerment.
