Role Overview
We are seeking an experienced Social Worker to join our team, working with adults who have learning and physical disabilities. This is an excellent opportunity for a qualified professional to contribute to our Adult Learning Disabilities and Physical Disabilities Team and make a positive difference in people's lives.

About the Position
This is a full-time role involving assessing the needs of adults with learning and physical disabilities, developing care plans, and collaborating with external agencies. The successful candidate will have a strong understanding of relevant legislation and the ability to work collaboratively within a multidisciplinary team. As a valued member of our team, you will receive comprehensive training and support to ensure your success in this rewarding role.

Responsibilities and Requirements
* Conduct assessments to identify the needs of adults with learning and physical disabilities.
* Develop and implement effective care plans that promote independence and quality of life.
* Collaborate with health professionals and other agencies to ensure comprehensive support.
* Hold a valid registration from Social Work England and possess relevant experience working with adults with learning disabilities and/or physical disabilities.

Benefits and Salary
The estimated salary for this role is £40,000 - £50,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including comprehensive training and a supportive work environment.
