Job Title: Mental Health Specialist

Salary: £35,000 - £45,000 per annum

This is an exciting opportunity for a qualified Social Worker to join our Adult Learning Disabilities and Physical Disabilities Team in Darlington. As a Mental Health Specialist, you will work closely with adults who have learning and physical disabilities, helping them to achieve greater independence, well-being, and access to tailored support services.

Key Responsibilities:

  • Conduct assessments to determine the needs of adults with learning and physical disabilities, and develop person-centered care plans that promote independence and quality of life.
  • Work collaboratively with health professionals, housing services, and other agencies to provide a coordinated approach to care.
  • Advocate for individuals to ensure their rights are respected, and that they have access to the services and resources they need.
  • Ensure compliance with relevant legislation, including the Care Act and Mental Capacity Act, and adhere to safeguarding procedures.
  • Support adults in transitioning to or maintaining independent living arrangements, including making referrals to appropriate community resources.

Requirements:

  • Qualified Social Worker with valid Social Work England registration.
  • Experience working with adults with learning disabilities and/or physical disabilities.
  • Strong understanding of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding practices.
  • Ability to work collaboratively within a multidisciplinary team.
  • Commitment to promoting equality, diversity, and inclusion in all aspects of work.
