Adults Social Worker Disabilities Team Lead

We Care Bespoke Ltd is a leading provider of bespoke care services. We are currently seeking a skilled Social Worker to join our team, working closely with adults who have learning and physical disabilities.
About the Role
This is an exciting opportunity for a qualified Social Worker to make a meaningful impact on the lives of adults with diverse needs. As a member of our Adult Learning Disabilities and Physical Disabilities Team, you will conduct assessments, develop person-centered care plans, and work collaboratively with health professionals to provide a coordinated approach to care.
Responsibilities
* Conduct thorough assessments to determine the needs of adults with learning and physical disabilities.
* Develop and implement effective care plans that promote independence and quality of life.
* Collaborate with health professionals, housing services, and other agencies to ensure comprehensive support.
* Advocate for individuals' rights and access to necessary services and resources.
* Ensure compliance with relevant legislation and adhere to safeguarding procedures.
Requirements
* Qualified Social Worker with valid registration from Social Work England.
* Experience working with adults with learning disabilities and/or physical disabilities.
* Strong understanding of relevant legislation, including the Care Act and Mental Capacity Act.
* Ability to work collaboratively within a multidisciplinary team and promote equality, diversity, and inclusion.
Salary and Benefits
The estimated salary for this role is £35,000 - £45,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including training opportunities and a supportive work environment.
