We are seeking a highly skilled and experienced Primary Care Mental Health Practitioner to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a key member of our primary care team, you will work closely with GPs, mental health professionals, and other stakeholders to provide high-quality mental health services to individuals in need.
Key Responsibilities- Work alongside mental health liaison practitioners to facilitate mental health assessments in primary care and support GPs with joint decision-making regarding secondary care referrals, signposting, and transition of service users from secondary mental health services back to primary care.
- Provide support, education, and problem-solving approaches for adults of working age, helping to inform GP management plans for those with mental health needs in primary care.
- Develop close relationships with primary care practices, attending practice meetings and maintaining regular informal contact to ensure seamless collaboration and effective service delivery.
- Registered mental health professional with significant post-qualifying experience in the relevant care group.
- Completion of mentorship/preceptorship course or equivalent, or intention to complete.
- Evidence of continuous professional development.
