About the Role
This is an exciting opportunity for an individual interested in palliative and end-of-life care to join our dynamic specialist team at St Luke's Hospice Plymouth. We seek an enthusiastic, highly motivated Registered Nurse to join our multi-disciplinary team in providing specialist palliative care to patients and their families.
Main Responsibilities
- Manage patients with complex needs, assessing and planning care to meet practical, psychological, spiritual, and social needs.
- Work closely with primary and secondary care professionals and Hospice colleagues, making good communication and teamwork essential.
- Collaborate with other Clinical Nurse Specialists, a Senior Healthcare Assistant, and a Hub Clinical Lead to provide comprehensive care.
- Develop and implement individualized care plans to improve the quality of life for patients with life-limiting conditions.
Requirements
- Registered Nurse with experience in palliative care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to assess and plan care for patients with complex needs.
- Knowledge of palliative care principles and practices.
