Job Summary

We are seeking a highly skilled and experienced Senior Clinical Nurse Specialist to join our Learning Disability Team in Plymouth. As a key member of our multidisciplinary team, you will be responsible for ensuring clinical effectiveness and coordinating services for individuals with complex learning disabilities.

Key Responsibilities
  • Assess, plan, and manage risk of individual's care needs, including physical, mental health, and behavioral challenges.
  • Case management and care coordination for individuals who are fully health-funded.
  • Liaise with commissioners and other agencies to ensure reasonable adjustments are made.
  • Collaborate with other teams to ensure equitable healthcare for individuals with learning disabilities.
  • Develop and implement care plans that prioritize individual needs and goals.
  • Provide expert clinical guidance and support to colleagues and other healthcare professionals.
  • Contribute to the development and implementation of policies and procedures related to learning disability care.
Requirements
  • Registered Nurse with a DipHE or Degree in Nursing and 3 years of post-registration experience.
  • Professional registration and evidence of continuing professional development.
  • Experience of working in multidisciplinary teams and across health and social care.
  • Ability to work autonomously and prioritize tasks effectively.
  • Excellent communication and interpersonal skills.
  • Ability to work with individuals who display behaviors of concern.
