Position Overview

We are seeking a skilled Clinical Nurse Specialist to join our dedicated team at Stoke Surgery, working on a part-time basis of 2-3 days per week under a Permanent Contract. This role is integral to our Primary Healthcare Nursing team, focusing on the provision of safe and effective nursing care to our diverse patient population.

Key Responsibilities

The responsibilities of the Clinical Nurse Specialist include, but are not limited to:

  • Conducting Cervical Screening and administering Childhood Immunisations in accordance with established patient group directives.
  • Providing Wound Care and offering Travel advice and immunisations.
  • Delivering Contraceptive advice and monitoring Hormone Replacement Therapy (HRT).
  • Performing ECGs and Phlebotomy procedures.
  • Managing Chronic diseases and conducting Doppler testing.
  • Chaperoning patients and promoting Health initiatives.
  • Carrying out Urinalysis as required.

About Our Practice

Stoke Surgery is a well-established GP practice, committed to serving our community with a patient base of approximately 8000 individuals. Our nursing team consists of three experienced Nurses, supported by a knowledgeable Healthcare Assistant. Additionally, we have two Paramedics and a Clinical Pharmacist who collaborate closely with our team.

Core Duties

The following are the essential duties of the Clinical Nurse Specialist:

  • Develop and implement health promotion and wellness initiatives.
  • Create and assess individual treatment plans for patients with chronic conditions.
  • Identify and support patients at risk of developing long-term health issues, aiming to prevent negative health outcomes.
  • Provide routine nursing care in line with clinical evidence, NICE guidelines, and the National Service Framework.
  • Conduct chronic disease clinics, delivering necessary patient care and referring to secondary or specialist services as needed.
  • Maintain accurate clinical documentation in compliance with current legislation.
  • Keep chronic disease registers updated.
  • Prioritise health concerns and intervene as appropriate.
  • Assist the team in managing clinical emergencies.
  • Implement vaccination programmes for both adults and children.
  • Guide patients in the use of prescribed and over-the-counter medications within the scope of practice.
  • Support the clinical team with safeguarding matters, adhering to local and national policies.
  • Communicate effectively with patients and their families, adapting methods to accommodate varying levels of understanding and cultural backgrounds.
  • Recognise and operate within personal competence and professional standards as regulated by the Nursing and Midwifery Council (NMC).
  • Deliver opportunistic health promotion when suitable.

Candidate Profile

Experience Requirements

Essential

  • Proven experience working within a nursing team.
  • Proficiency in recording precise clinical notes.
  • Understanding of the significance of evidence-based practice.
  • Knowledge of clinical governance principles.
  • Ability to operate within personal scope of practice and recognise when to refer to a GP or other team members.
  • Familiarity with Patient Group Directions (PGDs).

Desirable

  • Experience in Primary Care settings.
  • Background in General Practice nursing.
  • Flexibility to cover for colleagues during absences.

Qualifications

Essential

  • Fully Qualified Registered Nurse with a valid NMC registration PIN.

Desirable

  • Experience in General Practice nursing.
  • Background in chronic disease management.
  • Non-medical prescribing qualifications.
