About the Role

This is an exciting opportunity for an individual interested in palliative and end-of-life care to join our dynamic specialist team at St Luke's Hospice Plymouth. We seek an enthusiastic, highly motivated Registered Nurse to join our multi-disciplinary team in providing specialist palliative care to patients and their families.

Main Responsibilities

  • Manage patients with complex needs, assessing and planning care to meet practical, psychological, spiritual, and social needs.
  • Work closely with primary and secondary care professionals and Hospice colleagues, making good communication and teamwork essential.
  • Collaborate with other Clinical Nurse Specialists, a Senior Healthcare Assistant, and a Hub Clinical Lead to provide comprehensive care.
  • Develop and implement individualized care plans to improve the quality of life for patients with life-limiting conditions.

Requirements

  • Registered Nurse with experience in palliative care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of palliative care principles and practices.
