Job Summary

This is an exciting opportunity for an individual interested in palliative and end-of-life care to contribute to our dynamic specialist team. The temporary post covers maternity leave, providing a unique chance to make a significant difference in the lives of patients and their families.

Main Responsibilities

The Clinical Nurse Specialist will work within a Clinical Hub structure, collaborating with other Clinical Nurse Specialists, a Senior Healthcare Assistant, and a Hub Clinical Lead. Key responsibilities include:

  • Managing patients with complex needs, assessing and planning care to meet practical, psychological, spiritual, and social needs.
  • Working closely with primary and secondary care professionals and Hospice colleagues, emphasizing the importance of effective communication and teamwork.
  • Improving the quality of life for patients with life-limiting conditions, making a positive impact on their well-being and that of their families.
