Job Title: Clinical Nurse Specialist

An exciting opportunity has arisen for a Clinical Nurse Specialist to join our team at the DN Treatment Clinics/Lower Limb Service (LLS) working across Livewell Southwest. The LLS has been successful in becoming a National Wound Care Strategy Programme (NWCSP) Implementation Site 1st tranche for the NWCSP Lower Limb Recommendations.

The successful candidate will be responsible for providing specialist care in all aspects of lower limb health and skin care, working clinically under the guidance of Community Sisters and Team Managers. Health promotion and patient education are essential parts of this role, as is collaboration with other allied healthcare professionals.

The DN Treatment Clinics provide care to non-housebound patients that is unavailable at their GP surgeries: pleural drain management, urinary catheter management, routine tracheostomy care, nephrostomy care, and Topical Negative Pressure Therapy wound dressings.

Main Duties of the Job

This post holder will work as a member of the DN Treatment Clinics/Wellbeing Lower Limb Service and may be required to work independently during their working day.

The post holder is supported by the Community Team Manager and Specialist Service Lead and is responsible for providing specialist advice and information for the holistic assessment and management of Lower Limb disease.

Job Responsibilities
  1. To provide holistic evidence-based/best practice care to patients in accordance with National and Trust approved policies/procedures and individual care plans.
  2. To be responsible for specialist assessment, planning, implementation, evaluation of care packages on a day-to-day basis with support from the Clinical Nurse Specialist/Team Manager/Team Leaders.
  3. To problem solve and liaise as soon as possible with the Community Team Manager/Team Leaders any problems that may arise.
  4. To maintain accurate records, which are confidential, contemporaneous, legible, and all care given to be documented.
  5. To undertake comprehensive risk assessments of all situations associated with the care of patients to ensure nurses' and patient safety.
  6. To comply with all infection control policies and guidance, attend relevant updates, and report issues of concern to their immediate line manager.
  7. To act as a clinical advisor on health care issues within their area of expertise and act as a Link Nurse.
  8. To identify areas of improvement of practice and communicate to the Community Team Manager/Team Lead.
  9. To undertake health promotion as requested to all age groups and contribute to planned health promotion activities.
