Clinical Nurse Specialist

We are seeking an experienced Registered Nurse to join our Discharge to Assess team at Livewell Southwest. As a key member of our team, you will be responsible for providing high-quality nursing care to patients referred from the acute general hospital setting.
About the RoleThis is an exciting opportunity to work in a dynamic and multi-skilled team, consisting of nurses, social workers, occupational therapists, physiotherapists, community care workers, and support workers. Our purpose is to support a safe and timely hospital discharge of people who are medically fit and no longer need hospital care.
Key Responsibilities- Provide holistic evidence-based care to patients in accordance with national and organizational policies and procedures.
- Assess, plan, implement, and evaluate patient care on a day-to-day basis as part of a multi-professional team.
- Work autonomously and unsupervised in the community setting, recognizing the risks associated with lone working, aggressive behavior, and working proactively to reduce those risks.
- Liaise with planned resources, both internal and external to the organization, and work in partnership to offer optimum opportunities for people to meet their desired outcomes.
- 2+ years of recent experience as a Band 5 Nurse in an acute or community setting.
- Ability to demonstrate transferable skills.
- Experience or understanding of community working.
- Relevant clinical experience and competence, including medicines management and complex care coordination.
