Community Mental Health Nurse

We are seeking a highly motivated and caring professional to join our Weymouth and Portland Community Mental Health Team as a Mental Health Practitioner.
As a Care Coordinator, you will work closely with patients suffering from Serious Mental Illness, providing person-centred interventions to aid recovery. You will have a working knowledge of current bio psycho-social models of care and understand how to implement such interventions with your patients.
The successful candidate will be part of an integrated Multi-Disciplinary Team, consisting of Consultant Psychiatrists, Nurses, Occupational Therapists, Social Workers, and other healthcare professionals.
The role is full-time, permanent, covering the Weymouth and Portland area. A valid driving licence and use of a car are required for regular patient reviews and monitoring.
