As a Mental Health Advocate, you will play a vital role in addressing these needs and empowering individuals to take control of their lives.

About the Role:

\

This is a challenging yet rewarding opportunity to make a positive impact on the lives of vulnerable individuals. As an Outreach Caseworker, you will work in the community to identify and address unmet social needs that may be exacerbating physical and/or mental health.

\

Your Key Responsibilities:

\

You will work closely with clients to understand their unique needs and develop tailored support plans. This will involve building strong relationships with partners and stakeholders to lower stigma within the community and change mindsets and behaviours.

\

Requirements:

\

To be successful in this role, you will need experience in person-centred support planning and dynamic risk assessment. You will also possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

\
