Our Community Mental Health Team for Older People in Weymouth is seeking a dedicated Band 6 practitioner to join our well-established team.
We work with a multitude of services across Weymouth and Portland to ensure the mental health needs of the older adult population are met. As a care coordinator, you'll provide person-centred, holistic care to aid patients' wellbeing and recovery. Your role will involve assessing, planning, implementing, and evaluating programmes of care while prioritising and meeting deadlines, as well as supervising, teaching, and assessing others.
We offer a generous relocation package of up to £5,000 for those interested in moving to Dorset. This is a fixed-term role, and we welcome applications from Registered Mental Nurses with valid registration and evidence of continuous professional development.
