Older Adults Support Worker

The Dorset HealthCare University NHS Foundation Trust is seeking a highly skilled Older Adults Support Worker to join our Community Mental Health Team in Weymouth. As a Band 6 practitioner, you'll act as a dedicated care coordinator, providing person-centred care to aid patients' wellbeing and recovery.
Your role will involve assessing, planning, implementing, and evaluating programmes of care, prioritising and meeting deadlines, supervising, teaching, and assessing others. We work closely with local services, including the Crisis and Home Treatment Team and Intermediate Care Service for Dementia.
