Key Responsibilities

This Older Adult Healthcare Coordinator role requires the ability to work collaboratively with carers, professionals, and other healthcare teams to deliver holistic care to patients. You will be responsible for coordinating care plans, conducting assessments, and providing guidance on mental health issues.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. Knowledge of local mental health charities and peer-run services is advantageous, but not essential.

We offer a competitive salary of £35,392 - £42,618 per annum (pro rata for part-time), as well as a supportive and inclusive work environment that values staff development and well-being.
