Community Care Coordinator

Pick for Britain invites applications for a Mental Health Practitioner to join our well-established Community Mental Health Team in Weymouth and Portland.
This is a Full-Time permanent position, requiring a valid driving licence and use of a car for reviewing patients in their own homes and community settings.
As a Mental Health Practitioner, you will provide targeted interventions to aid recovery for patients with Serious Mental Illness, working as a Care Coordinator for a dedicated caseload.
The team operates collaboratively, offering opportunities for skill development in therapeutic interventions and nursing practices.
You will work closely within our Integrated Multi-Disciplinary Team, consisting of Consultant Psychiatrists, Mental Health Nurses, Occupational Therapists, Social Workers, Advanced Nurse Practitioners, Support Time Recovery Workers, Physical Health Workers, and our Psychology Team.
The estimated annual salary for this role is £46,113 - £61,449, based on experience and qualifications.
