About the Role

This Active Care Coordinator role is an exciting opportunity to make a positive impact in the life of a young person.

You will work closely with the child, their family, and other professionals to create a safe and nurturing environment that fosters growth and development.

The role involves organising and participating in engaging activities, such as walking, paddle boarding, and other outdoor pursuits, to promote physical and emotional well-being.

You will also be responsible for developing and implementing individualised care plans to meet the unique needs of the child.

Key Responsibilities
  • Organise and participate in engaging activities, including walking, paddle boarding, and other outdoor pursuits.
  • Develop and implement individualised care plans to meet the unique needs of the child.
  • Work closely with other team members, families, and external agencies to ensure seamless care and support.
  • Provide consistent care and support by working long days and ensuring stability for the child.
What We Offer
  • A competitive salary of £25,000 - £30,000 per annum, depending on experience.
  • A comprehensive induction and training programme, including a Level 4 Certificate in Therapeutic Care where required.
