Job Summary

We are seeking an experienced Community Health and Care Practitioner to join our team in the West Community Learning Disability Team. As a Community Health and Care Practitioner, you will be responsible for working with adults who have a learning disability, providing health assessment and therapeutic intervention, and supporting them to take control of their own health.

The successful candidate will be expected to work autonomously, using specialist clinical skills and knowledge to identify the health needs of people with a learning disability and develop effective care plans. You will also be required to work effectively within a multi-disciplinary team and communicate effectively with people who have a range of communication difficulties.

We are looking for a practitioner who is passionate about delivering high-quality care and has experience of working with adults who have a learning disability. The role will involve flexible working options, including rotation, and will require you to work effectively within a multi-disciplinary team.

Estimated salary: £34,893 - £44,141 per annum.
