We are seeking an exceptional Learning Disability Nurse to join our team at Norfolk Community Health and Care NHS Trust. In this role, you will work within a dynamic and supportive environment to provide high-quality care to individuals with learning disabilities.

You will be responsible for assessing care needs, developing individualized care plans, and collaborating with multidisciplinary teams to deliver person-centered care. The ideal candidate will possess a relevant degree in nursing, be registered with the NMC, and have experience of working with individuals with learning disabilities.

Key Responsibilities
  • To assess the health and social care needs of individuals with learning disabilities.
  • To develop and implement individualized care plans that promote autonomy and independence.
  • To work collaboratively with multidisciplinary teams to deliver person-centered care.
