Role Summary

We are seeking a skilled Clinical Case Manager to join our team as a Functional Specialist. In this role, you will work with clients to assess their health and disabilities, providing comprehensive and unbiased support.

Key Responsibilities:

  • Conduct face-to-face and virtual assessments to understand clients' health conditions and disabilities
  • Manage caseloads from end to end, including delivering initial review calls and paper-based reports
  • Collaborate with other healthcare professionals to ensure seamless support for clients

Requirements:

  • Qualified Nurse, Mental Health Nurse, Occupational Therapist, Physiotherapist, or Paramedic
  • 1+ years post-qualification experience
  • Valid registration on NMC or HCPC without restrictions

Benefits:

  • Competitive salary of £38,500 - £41,500
  • 5% retention bonus, rising to 10% quality and retention bonus
  • OTE: £45,650
  • Hybrid full-time, part-time working hours available
