About the Role:
As a Learning Disability Care Coordinator, you will be responsible for:
- Coordinating care plans, identifying individual needs, and developing personalized support strategies.
- Collaborating with healthcare professionals, family members, and other stakeholders to ensure comprehensive care.
- Maintaining accurate records, reporting incidents, and adhering to relevant policies and procedures.
Requirements:
