About the Role

We are seeking a skilled Community Registered Mental Health Nurse to join our Access and Assessment Team in Great Yarmouth and Waveney. As a key member of our team, you will be responsible for providing high-quality care to adults over the age of 25, working closely with our multidisciplinary team to assess and manage referrals into secondary care.

Main Responsibilities

Key responsibilities of the role include:

  • Undertaking assessments, planning, and delivering direct patient care with minimal supervision.
  • Communicating decisions about mental health care services, referrals, and future support arrangements to individuals with mental health care needs.
  • Accurately recording all patient support, including assessments, reviews, day-to-day contact, and discharges.
