About the Role
We are seeking a highly skilled Registered Nurse (Adult) to join our team at Gorleston Primary Care Home.
As a Community Staff Nurse, you will provide person-centered care to adults in their own homes, residential care, or clinic settings. You will work closely with senior nurses to gain clinical skills and eventually hold your own caseload of patients.
Main Responsibilities
- Conduct holistic assessments for patients referred to the service, including environmental, moving, and handling assessments, pressure area risk, and nutritional needs.
- Participate in the delivery of nursing interventions, such as urinary catheterization, administration of medication via injection, wound care, and palliation.
- Support patient self-care using health coaching techniques and work collaboratively with residential care home colleagues to promote high standards of care.
- Contribute to the clinical governance agenda through participation in clinical risk assessment and clinical audit.
- Work as part of the multidisciplinary team to deliver planned and unplanned services, working in partnership with senior members to review delegated caseloads and ensure high-quality care.
- Plan and organize complex clinical activities in community settings, ensuring best practice is delivered to patients.
- Maintain legible, accurate, and contemporaneous patient records, ensuring compliance with ECCH Policy and Nursing and Midwifery Council standards.
Requirements
- Professional Registration with Nursing and Midwifery Council (NMC) Registered Nurse Level 1 (Adult)
- BSc in Nursing Practice or equivalent
- Excellent communication, interpersonal, and record-keeping skills
- Competent IT skills and ability to work unsupervised
- Previous experience in healthcare, with recent clinical experience in community nursing desirable
