About the Role

We are seeking a skilled Community Registered Mental Health Nurse to join our Access and Assessment Team in Great Yarmouth. As a key member of our team, you will play a vital role in supporting the local community and contributing to the development of our services.

Key Responsibilities

  • Undertake assessments, planning, and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.
  • Communicate decisions about mental health care services, referrals, and future support arrangements to people with a mental health care need.
  • Be responsible for accurate recording of all patients' support, including assessments, reviews, day-to-day contact, and discharges.
