Mental Health Liaison Practitioner

About the Role
We are seeking a skilled and experienced Mental Health Liaison Practitioner to join our dedicated team at the James Paget University Hospital in Gorleston.
As a Mental Health Liaison Practitioner, you will play a vital role in supporting individuals attending the hospital with new or ongoing mental health difficulties and illness. Your key responsibilities will include identifying their needs, ensuring these are met effectively, and contributing towards timely and supportive discharge planning.
This is a fantastic opportunity for someone who is motivated, compassionate, and a good team player, with a willingness to make a difference in the lives of adults and older people experiencing mental health crisis.
Key Responsibilities
- Work closely with colleagues in the A&E department, wards, and other mental health professionals to provide a proactive interface between mental health and acute services.
- Support individuals presenting to the acute hospital with mental health issues, including those with complex needs and risk management requirements.
- Contribute to the development and implementation of discharge plans, ensuring a smooth transition for patients back into their communities.
Requirements
- Nursing degree or equivalent, DIP OT, Clinical Psychology, or equivalent.
- Effective risk assessment and risk management experience.
- Minimum 1 year experience at B6, with additional post-registration qualifications desirable.
