Salary

The estimated annual salary for this role is around £40,000 - £60,000, depending on experience and qualifications.

Job Description

We are seeking a highly skilled Senior Community Mental Health Practitioner to join our team. The successful candidate will work closely with young people under 18 years old, residing in Norfolk or Suffolk, to provide intensive support and prevent hospital admissions.

The role involves conducting individualized risk assessments, developing personalized care plans, and working collaboratively with professionals from various agencies to ensure seamless transitions of care. Our practitioner will also be responsible for implementing therapeutic interventions, promoting positive futures, and empowering young individuals to manage their mental health effectively.

Required Skills and Qualifications

To be considered for this position, you must possess:

  • A registered qualification as a Mental Health Nurse, Social Worker, or Occupational Therapist.

Desirable qualities include experience working at a senior clinical level, knowledge of quality standards, and the ability to travel independently between Trust sites.
