Job Overview

We are currently seeking a skilled Community Mental Health Nurse to join our team in South West Shropshire. As a key member of our multi-disciplinary team, you will be responsible for assessing the care needs of individuals with mental health-related difficulties and developing relevant plans to support their recovery.

Main Responsibilities

  • Taking responsibility for the management of a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans.
  • Participating in the setting of quality standards, auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  • Administering medication, complying with relevant trust procedures and NMC Guidance.
  • Assessing patient's individual care needs, developing, implementing, and evaluating programmes of care.
  • Delivering a range of interventions in accordance with the agreed plan of care and pathway guidance, including pharmacological and psychologically informed interventions.

Requirements

  • Registered Mental Health Nurse on the NMC Register.
  • Experience of working with people experiencing mental health-related difficulties.
  • Ability to work autonomously, as part of a multi-disciplinary team.
