Community Mental Health Nurse Specialist

We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care and support to individuals with mental health needs in the community.
Key Responsibilities- Assess and manage complex caseloads, including specialist assessments, care planning, and evaluation of individual treatment plans.
- Participate in the setting of quality standards, auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
- Administer medication, complying with relevant Trust procedures and NMC Guidance.
- Deliver a range of interventions, including pharmacological and psychologically informed interventions, to support individuals in achieving their goals.
- Complete and contribute to robust risk assessments and risk management plans, promoting positive risk-taking to improve quality of life and maximize occupational performance/independence.
- Maintain an effective reporting system, observing and reporting verbally and in writing on patient conditions.
- Utilize agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
