About the Role

We are seeking a highly skilled and motivated Community Mental Health Nurse to join our team at Midlands Partnership NHS Foundation Trust. As a Community Mental Health Nurse, you will be responsible for assessing the care needs of individuals with mental health-related difficulties and developing relevant plans to support their recovery.

Main Responsibilities

Key duties and responsibilities include:

  • Assessing the care needs of individuals with mental health-related difficulties and developing relevant plans to support their recovery.
  • Providing nursing interventions to support individuals to meet their agreed goals and coordinate and/or participate in the review of progress towards those goals.
  • Participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Administering medication, complying with relevant Trust procedures and NMC Guidance.
  • Assessing patient's individual care needs, developing, implementing and evaluating programmes of care.
  • Delivering a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
  • Maintaining an effective reporting system by observing and reporting verbally and in writing on patient conditions.
  • Utilising agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
